-Justification-
Ayli, Eternal Pilgrim, is getting cut for the same reason Disciple of Griselbrand was cut- this deck doesn't generate a lot of creatures to sacrifice. While her second ability is powerful, Karlov already has creature-removal built in. The deck also has cards like Solemn Offering and Anguished Unmaking to remove other kinds of permanents in her stead.
Filigree Familiar is being added because, at 3 mana, it hits the battlefield the first turn Karlov can attack. This makes Karlov into a 4/4 on turn 3 (when on curve), and adds 2 counters anytime in the late-game. The card-draw trigger also makes it a great blocker while Karlov is tapped from attacking.

[IN] Claws of Gix
-Justification-
Both of these cards are great in this deck, and serve the role of providing multiple life-gain triggers, at once, for a large sacrifice. They each have their pros and cons, but why Claws of Gix over Reap the Rewards? Claws of Gix doesn't require white mana to cast, so all of my lands can tap, and be sacrificed, to gain life repeatably. It also has more potential when I have creatures on the battlefield, as they can be sacrificed to save my mana-base for future turns. Where Claws of Gix is worse is if the meta has a lot of Krosan Grips. Additionally, losing the ability to keep this type of card hidden until needed (instant-speed) may make this change temporary.

[IN] Vesuva
-Justification-
Shambling Vent was rarely activated as a creature. Most of the time, I'd rather be casting impactful spells over making a 2/3 with Lifelink. Vesuva will most likely be a black-white dual-land when it enters the battlefield anyways. If it doesn't, it has the up-side of becoming an opponent's utility-land in the late-game.

[OUT] Exquisite Blood
[IN] Swamp
-Justification-
Exquisite Blood isn't great without the other combo-piece (Sanguine Bond). Yes, Vizkopa Guildmage is in this deck, but they rarely saw each other. At 5 mana, this enchantment doesn't do enough at the top of this deck's curve to really want to cast it over other options.
A swamp takes it's place, as I've been drawing too many opening-hands with 1 land.
