Any idea how this user got his avatar to ignore the standard avatar dimensions? Is it just a bug?

That just happens, I uploaded my avatar and did the same thing without asking...

This is happening since the site was moved to the new servers.
I belive some component is not installed on the server for resizing images?
This worked fine on the old servers...

This was indeed a missing component bug on the new servers, exactly like wardog has guessed :).

It should now be fixed for all new avatars uploaded, but some of the old ones are still bad. I've fixed Jodahae's. If anyone finds more of them that slipped, feel free to post here, I'll manually fix.
